Submission Forms and Permits
Need a form? This page
allows you to download and print the required documents for your
DNA testing needs. To download a document, please follow these simple
instructions:
1. The documents are
in pdf format and require Adobe
Acrobat Reader. If you do not have Adobe Acrobat Reader installed,
please download from Adobe's website and follow their installation
instructions.
2. Click on the document
you wish to download. After several seconds the document should
appear in an Acrobat Reader window from where the document can then
be printed from this window
If for any reason you
are unable to download or print these documents, please contact
us and we will be happy to fax or mail you any forms or permits
you require.
Submission Form (Domestic
use only)
To insure that we receive
all the information necessary to process your order, please include
a completed Submission Form with each of your shipments.

U.S. Will not accept samples from other
countries without proper permits!! Please, send
all non U.S. samples to our UK office for
processing. This will prevent unintentional
errors in paperwork from turning into astronomical fines for us.

As part of our ongoing
efforts to provide you with the finest and most reliable testing
for birds, we would like to remind our customers outside the United
States and its territories that all samples submitted to Avian Biotech
MUST be accompanied by the
following documentation. Failure to do so
could lead to thousands of dollars in fines for us:
1. A USDA Import permit
(provided by Avian Biotech)
2. A U.S. Fish and Wildlife
permit (provided by Avian Biotech)
3. A submission form
(provided by Avian Biotech) listing the species name and origin
of each sample.
4. Cites Export permits
are required for all Appendix I, II and III species of birds. Export
permits can be obtained from your management authority. To find
the local management authority office in your area, please visit

Authority Directory.
